Incident Report

This report must be completed by a SHINE staff member immediately following any incident involving:


  • Illness or injury to a member, guest, or staff

    • this includes any members or guests that have dizzy spells, wherein juice boxes are handed out

  • Theft or suspected theft

  • Damage to SHINE property or equipment (i.e. speakers, computers, phones, or any property with a retail value over $100)

  • Damage to studio/building where property management would need to be called/was called (i.e. flood, leak, plumbing issue, snow accumulated, humidity problem, power outage etc)

  • Disruptive behaviour, safety concerns, or security threat

  • Any other unusual event (i.e. not regularly occurring in a normal day of operations) requiring documentation


Please fill out all sections with as much detail as possible while on shift and submit to the manager on duty or studio owner before the end of the applicable shift.
